Application for Instrumental Lessons
If your child wishes to take up instrumental lessons at Brooklands Campus, please read all the information on the Music Tuition Policy and complete the Music Tuition Application Form.
You must complete this form for each child and each instrument they wish to take lessons in. Therefore, one child wishing to take up two instruments would require this to be completed twice.
Request to End Instrumental Lessons
Should your child no longer wish to continue with instrumental lessons, please complete and submit the Music Tuition Cancellation Form.
